We are looking for an enthusiastic and committed Service Delivery Coordinator who shares our values to join our team in Derby.

We currently looking for Service Delivery Coordinator:
The role is responsible in developing and maintaining strong, positive relationships with individuals we support and their families. This partnership approach will help to set and achieve goals in a person and family-centred way.
Duties may include, but are not limited to:
Demonstrate understanding and respect of individuals rights to exercise choice and control in the delivery of supports.
Ensuring the plans meet all Qualitative Criteria for the Quality Management Framework and all reporting requirements to NDIA
Reviewing and monitoring individual plans, seeking regular feedback from staff and the individual, their family and carers to update or change strategies where required.
Provide mentoring and coaching to support staff to ensure delivery of high quality services
Providing supports to participants
Essential Requirements
Demonstrated values driven and person centred-approach to provide support for people with disabilities, their families and carers.
Demonstrated knowledge and understanding of the National Standards for Disability Services.
Experience in planning, coordinating and monitoring supports and services for people with disabilities, their families and carers.
Demonstrated ability to lead a work group, set priorities and monitor work flows.
Demonstrated high level of verbal, written and / or communication skills.
Demonstrated ability to manage budgets.
Relevant qualification in Human Services, Community Services or Disability Sector.
A satisfactory National Police Clearance that is less than three months old.
NDIS Worker's Screening (or willingness to obtain).
