What your day will look like….

In this role, your day will be filled with meaningful connections and purposeful action. You'll work closely with families, helping them navigate their health journeys, advocating for their needs, and providing culturally safe and compassionate care. You'll coordinate case plans, conduct home visits, and link families with vital services in areas such as maternal and child health, chronic disease, and health promotion. You'll collaborate with a multidisciplinary team while mentoring others and sharing your cultural insight. With the freedom to work independently, you'll have the opportunity to influence real change – both in individual lives and across the wider health system – while staying connected to community, culture, and the values that matter most.
