Lead and support the administrative team to ensure the effective onboarding and ongoing coordination of Support at Home services for clients requiring case management.
What you’ll be doing:
- Serve as a point of contact for package management inquiries related to SAH clients, responding to individual residents, community agencies, and health professionals.
- Oversee and manage client correspondence, including Service Agreements, Introductory Letters, Cab Charge agreements, and outgoing mail preparation, ensuring accuracy and compliance.
- Oversee the package management functions, ensuring financial accuracy which includes but is not limited to fund reconciliations, and invoice processing, compliance with regulations and policies, and timely processing of client funding.
- Supervise and support the administration team in adding and updating client funding within the client management system, monitoring expenses, and ensuring clients receive accurate monthly expenditure statements.
- Oversee service navigation processes for all SAH clients with care management, ensuring compliance with service guidelines and adherence to established checklists.
- Provide support to clients, empowering them to navigate the aged care system, make informed choices, and access services that best meet their needs.
- Support the implementation of Continuous Quality Improvement and Occupational Health & Safety programs within Aged & Disability Services.
- Ensure compliance with organisational policies, procedures, and legislative requirements, including the Aged Care Act, Victorian Health Act, Privacy Act, and Freedom of Information obligations.
The skills and experience you’ll bring to the role:
- A strong understanding of client intake, service referrals, and assessment processes within aged care and disability services.
- Proficiency in managing, maintaining, and analysing administrative data to support operational efficiency and reporting.
- Familiarity with government-funded programs, aged care quality standards, and relevant compliance requirements.
- Demonstrating excellent written and verbal communication skills to engage with clients, colleagues, and external agencies.
- Building and maintaining positive relationships with internal and external stakeholders, including service providers, council staff, and community members.
- Responding to enquiries with professionalism, empathy, and efficiency, ensuring a high level of customer service.
Required Certifications & Qualifications:
- A Certificate or Diploma in Office/Business Administration or relevant equivalent qualifications.
