We're seeking a compassionate and experienced Care Manager who is passionate about delivering high-quality, person-centred care to join a close-knit team located in the heart of the Wheatbelt region of rural Western Australia. This is a permanent full-time role with the opportunity to make a meaningful impact in the lives of older Australians.

Working closely with the Facility Manager, you will lead the clinical team to ensure that residents' health and wellbeing are prioritised through best-practice nursing care, regulatory compliance, and a culture of continuous improvement. In a rural setting, your ability to think on your feet, mentor others, and foster strong relationships will be essential.

To be successful in this role, you'll bring solid clinical experience, a hands-on leadership style, and a commitment to building a warm and inclusive home environment. We're looking for someone who is solutions-focused, approachable, and confident leading a team in a regional aged care setting.

Key Responsibilities:

• Facilitate effective communication between nursing staff, care workers, allied health, and visiting GPs to ensure holistic care.
• Build trusting relationships with residents' families by keeping them informed and involved in their loved ones' care.
• Mentor and support nursing and care staff in clinical decision-making, care planning, and skills development.
• Maintain compliance with Aged Care Quality Standards and contribute to continuous improvement initiatives.
• Act as a clinical resource for the team, stepping in to assist with assessments, medications, and incident reviews when needed.

Skills, Attributes & Experience Required:

• Current registration with AHPRA as a Registered Nurse
• At least 3 years' experience in a senior RN or Care Manager role in aged care (regional/rural experience highly regarded)
• Demonstrated leadership and team-building capability
• A proactive, hands-on approach to clinical care and staff development
• Strong understanding of aged care accreditation and compliance
• Ability to problem-solve in a resource-limited rural environment
• Postgraduate study in aged care, dementia, or palliative care is desirable
