$37.53 to $43.45 + salary packaging | Grade 1 Social Worker - Horsham

Permanent Full Time | 40 hours per week including a monthly ADO

About the role
Join a dynamic and supportive Social Work team that values compassion, learning, and collaboration. Based at our Horsham campus, this Grade 1 role supports patients across Acute, Subacute, and Community programs, helping to address the psychological, social, and emotional impacts of illness and hospitalisation. You'll be part of a broader network of over 40 Social Workers across Grampians Health, and have access to a range of structured supports including in-service training, Grade 1 supervision, external professional development, and study leave. If you're passionate about person-centred care and developing your career in health, this is an ideal opportunity.

Key responsibilities

• Deliver Social Work services across acute, subacute and community settings
• Complete psychosocial assessments and care planning for patients
• Provide grief and bereavement support to patients and families
• Liaise effectively with internal teams and external community agencies
• Participate in clinical supervision, service development, and quality activities
• Support clients and teams with informed decision-making and advocacy

Skills and experience

• Eligible for AASW membership (Bachelor or qualifying Master's in Social Work)
• Demonstrated knowledge of psychosocial assessment and care planning
• Strong communication and interpersonal skills
• Ability to work independently and as part of a multidisciplinary team
• Understanding of hospital systems and community resources
