Registered Nurse – Chronic Disease Care & General Practice

🩺 Casual/Part-Time Position

Our growing practice is looking for a dedicated Registered Nurse to join our team. This role involves providing Chronic Disease Care as well as fulfilling general practice nursing duties.

About the Role

The successful applicant will coordinate care for patients with chronic illnesses, including:
✅ Conducting health assessments
✅ Planning and facilitating Care Plans
✅ Supporting and mentoring patients in self-management of their conditions

You will be supported by experienced staff as part of a collaborative and welcoming team. ThriveGP provides holistic and respectful healthcare, enabling both our patients and team members to thrive in their lives.

Essential Requirements

Registered Nurse with at least one year of nursing experience
A true team player with excellent communication skills
Strong organisational skills, initiative, and flexibility

Desirable (But Not Essential)

Experience in general practice or community nursing
Nurse Immuniser qualification

Training & Support: If you have the essential skills and a great attitude, we will provide education and upskilling to help you succeed in this role.
