We're an NDIS-registered provider, also registered for Community Nursing, proudly delivering

person-centred care across Hobart and surrounding areas.

What You'll Be Doing:

- Community Nursing with heart and purpose

- Continence assessments and complex care planning

- Supporting participants with mental health, diabetes, and medication management

- Developing tailored care plans and monitoring outcomes

- Educating and mentoring support staff

- Keeping participant records up to date and detailed

- Being a supportive presence for participants and their families

- Collaborating with allied health and other professionals

What We're Looking For:

- Registered Nurse with current AHPRA registration

- 2+ years of experience (community, maternal, or child health preferred)

- Strong clinical knowledge and decision-making skills

- A caring, energetic, and proactive approach to participant care

- Confidence to work independently and contribute to a team

- Driver's licence and reliable transport
