The Role:

This is role is a full-time Service Coordinator located in Welshpool, WA. As a Service Coordinator, you’ll play a key role in supporting participant onboarding, building strong relationships with participants and their support networks, and ensuring services are delivered in line with individual goals and NDIS requirements.

This is a fast-paced, participant-focused role that involves coordination, planning, and problem-solving. You’ll work closely with participants, families, Support Coordinators, and our internal rostering team to make sure supports are delivered smoothly and consistently.

Key Responsibilities

Supervise day-to-day operations across multiple supported homes

Support service reviews, participant assessments, and Intake processes.

Provide ongoing mentoring, guidance, and performance support to House team leaders and Support Workers

Ensure services align with individual care plans and NDIS guidelines

Monitor and manage NDIS funding, budgets, and participant outcomes

Conduct regular home visits, audits, and quality assurance checks

Liaise with families, health professionals, and other stakeholders

Oversee rosters, incident reports, documentation, and service compliance

Contribute to strategic planning, staff recruitment, training, and development

Create and maintain a culture of safety, professionalism, and client-first service

Restrictive Practices Reporting on PRODA and Authorization Process.

It is an office based on site role. There is a requirement to travel for work purposed as needed.

Qualifications & Experience:

Essential:

Diploma of Community Services or equivalent qualification.

Applicant must be Permanent Resident or Australian Citizen.

Previous experience in the disability sector.

Excellent organisational and administrative skills.

Strong written and verbal communication skills.

Ability to multitask and manage competing priorities.

Passionate about disability care with a willingness to learn and grow.

Desirable:

Knowledge or previous experience with NDIS processes, compliances and guidelines.

Previous experience in Community services or Disability Services.
