The Home Care division helps older Australians remain independent in their own homes through personalised, high-quality supports. About the Role
We’re seeking a passionate and experienced Team Leader to lead our Home Care Packages (HCP) team, including both clinical and non-clinical staff. This role is perfect for someone currently working as an experienced HCP Coordinator ready for a step up—or a seasoned supervisor looking for a meaningful leadership opportunity. As the Team Lead you will ensure superior customer service and delivery of client focused care and services that effectively meets their individual needs.
As the HCP program transitions to the Support at Home Program in July 2025, you’ll play a pivotal role guiding your team and clients through the change. Key Responsibilities

  • Lead and support a team of HCP Coordinators, RNs and ENs to deliver person-centred care
  • Oversee daily service delivery, performance management and incident resolution
  • Ensure quality care aligned with Aged Care Quality Standards and client goals
  • Monitor and manage team capacity, resource allocation and client budgets
  • Drive client growth, satisfaction and retention
  • Liaise with clients and families to develop, review and monitor individual care plans
  • Prepare operational reports and contribute to continuous improvement
  • Support staff through sector reforms including Support at Home transition

Skills and Experience
Essential Criteria:

  • Ideally tertiary qualifications in Related Discipline or Nursing
  • Demonstrated experience in community based aged care
  • Proven empathetic leadership experience in a clinical or community setting
  • Strong interpersonal and communication skills
  • Budgeting and financial management experience
  • Strong understanding of HCP guidelines and Aged Care Quality Standards

Mandatory Requirements:

  • National Police Check
  • Current Driver’s Licence & access to a vehicle
  • Up-to-date COVID-19 and influenza vaccinations
