The exciting opportunity:
- One year parental leave contract
- Work across Perth’s Northern suburbs coordinating 40 home care clients, promoting healthy ageing and increasing their ability to stay at home
- Lead and manage your team of Support Workers and provide supervision and support to students and volunteers
- Conduct assessments and create, review and update individualised care plans that empowers the elderly to meet goals and maintain independence
- Ensure suitable support services are provided that will improve quality of life and facilitate community re-engagement
- Monitor and manage client budgets ensuring they comply with their approved package levels and individual requirements
- Work alongside the scheduling and service team to help clients navigate the aged care services
- Develop and maintain mutually beneficial relationships with external stakeholders so clients have access to numerous social opportunities and lifestyle options

- $85k - $95k + super + salary packaging
About you:
- Experienced as a Case Manager or Service Coordinator of home care clients
- Strong assessment and care planning skills, providing services that optimise client budgets
- Committed to using a client directed model of care to empower clients and encourage reablement and community engagement
- Fantastic customer service and negotiation skills with a proven track record building and maintaining relationships with clients
- Energetic and enthusiastic leader who inspires and supports their team
