ABOUT THE ROLE
Reporting to the Operation Manager, this role provides leadership to focus the team on delivery of Person-Centred Active Support to tenants with a disability who reside in the verticals to live independently. The Senior Support Leader has a well-developed understanding of the Civic practice and processes.
This is a customer service focussed role! You will be required to build rapport with service coordinators, health professionals, the broader community, as well as your internal stakeholders to ensure we are providing the right support at the right moment to create moments that matter.
This is a full-time opportunity where you will lead by example providing hands on support when required as well building a team that support each other to provide continuity of services and ensuring that the client documentation is accurate and up to date.

DUTIES & RESPONSIBILITIES
Lead the team to deliver Person Centred Active Support according to the Practice Framework in all aspects of Client care.
Provide operational administrative support which can include roster support, performing site inductions, coordination of team meetings/case conferences etc.
Advocate for Clients, providing the right amount of liaison between families, guardians, community groups, healthcare professionals, teachers and other services in the individual’s life.
Ensure Support Workers receive thorough and appropriate induction enabling successful transition to supporting tenants with a solid understanding of their challenges and support requirements.
Mentor staff members on identified learning needs to support the team’s development and improve client outcomes.
Assist the Operation Manager to govern the budgets, administrative and financial systems and records of the program and monitor their use in accordance with the organisation’s requirements.
ESSENTIAL CRITERIA
Ideally will have a certificate III or IV in community services, disability, aged care or related field. You may also be studying towards a qualification in social work, nursing, allied health or similar.
If not qualified you will have exceptional customer service and communication skills to ensure that you can understand and problem solve tenants feedback and negotiate great service outcomes for all stakeholder.
Experience in the delivery of care outcomes for those with high complex needs.
Ability to build strong working relationships with different stakeholders such as clients, families, colleagues, leaders and allied health professionals.
Creativity to be able to work clients and team members to meet client goals.
A passion for creating incredible client experience s that allows people to realise their potential.

Civic provides a range of individualised supports to complete everyday personal activities, thereby assisting in the transition to more independent living. Experienced Civic staff use a Person Centred Active Support (PCAS) approach with a range of living activities.
Do you need assistance with day-to-day personal activities?
Civic can provide help with dressing, feeding, toileting, administration and monitoring of medicines. We can provide support with shopping, domestic assistance, gardening and home maintenance services and other day-to-day tasks.
