We're seeking a compassionate and driven Community Care Coordinator to deliver tailored ageing and disability care programs that help clients remain safely in their homes. Working within a multidisciplinary team, you'll identify needs, coordinate care plans, liaise with stakeholders, and evaluate outcomes that truly make a difference in people’s lives. Your work will span assessment, advocacy, service integration and improvement—ensuring culturally sensitive, inclusive, and responsive care for every client.

Key Responsibilities:

• Assess client needs and develop individualised care packages
• Coordinate and monitor access to health, community, and support services
• Engage in service evaluation, system improvements, and community consultation
• Advocate for clients while encouraging their independence
• Maintain accurate case files and contribute to data reporting and quality improvement
• Collaborate across health, government and community sectors

What We’re Looking For:

• Case management experience, especially in complex health or aged care
• Excellent communication, empathy and time management skills
• Experience working in multidisciplinary and culturally diverse environments
• Knowledge of primary health care principles and care planning
• A team player committed to quality care and continuous improvement
