Nurse Care Manager – Community

We are a small, locally run home care organisation providing support to older adults living in their homes.

Our mission is simple: to make a positive difference in the lives of our clients by delivering care that’s compassionate, personal, and empowering. Due to continued growth, we’re looking for a Registered Nurse to join our team as a Nurse Care Manager.

We're seeking someone who takes a hands-on approach, enjoys client engagement, and thrives in a role that combines clinical leadership with coordination and relationship-building.

About the Role:

As a Nurse Care Manager, you’ll play a central role in coordinating and delivering person-centred care. You’ll work closely with clients, families, and support staff to assess needs, develop care plans, and oversee the delivery of high-quality services at home.

This role includes both clinical and case management responsibilities, as well as supporting our wonderful team of care workers.

Key Responsibilities:

Conduct clinical assessments and create tailored care plans

Coordinate and oversee in-home care services

Build trusted relationships with clients and their families

Collaborate with GPs and allied health professionals

Ensure compliance with aged care standards and best-practice care

Support onboarding of new clients and regular care reviews

Manage care budgets and maintain accurate documentation

Actively seek feedback and manage service expectations

What You’ll Need:

Current AHPRA registration as a Registered Nurse

Strong experience in Wound and Catheter care

Advanced Clinical assessment skills chronic disease management

Experience in aged care, community nursing, or similar

Strong clinical judgment and decision-making skills

Excellent communication and coordination abilities

Current Police Clearance, First Aid, and CPR (or willingness to obtain)

Valid driver’s license and access to a reliable vehicle
