Position description

Cross Care Group (CCG) is extending its “hospital-to-home” services to Bribie Island and needs warm-hearted, switched-on Support Workers to help clients in Bongaree, Woorim, Banksia Beach and neighbouring suburbs live well where they belong—at home and in their community.

You’ll team up with our nurses and allied-health crew to deliver hands-on daily support, community participation and, for some clients, high-intensity disability care.

Location: Bribie Island & surrounds, QLD

Training: Paid induction + buddy shifts

Start date: Immediate once onboarding checks clear

The role – what a typical week looks like

Personal care: showering, grooming, continence support

Daily living: meal prep, light housekeeping, medication prompts

Community participation: beach walks, hydrotherapy, shopping trips, social events

High-intensity supports (after training): hoist transfers, bowel care, PEG feeding, seizure protocols

Accurate shift notes & incident reporting on our mobile platform

Working alongside RNs, OTs and behaviour specialists to deliver person-centred plans

What you’ll bring

Certificate III or IV in Individual Support, Disability or similar—or substantial industry experience

Current First Aid & CPR, NDIS Worker Screening Check, QLD Blue Card

Open driver’s licence and reliable, insured vehicle

Tech confidence (shift notes & e-learning on your phone)
