About the Opportunity

We currently have an exciting opportunity to join the Metropolitan Youth Health (MYH) team as a Team Leader, where you will be responsible for:

  • Coordinate site service plans and align with MYH planning processes.
  • Collaborate with MYH leadership (including Aboriginal and Young Parent Program Leads), local government, and health services to address emerging needs and system issues.
  • Oversee allied health practices and the multidisciplinary team, ensuring consistency in practice and policy.
  • Provide expert clinical knowledge for effective therapeutic care, care coordination/case management and best practice delivery in line with the Model of Care.
  • Lead and manage MYH multidisciplinary team activities to maintain quality client care and drive change in a complex environment.
  • Identify, plan, deliver and document projects, which provide a forum for the discussion of health issues and build young people’s capacity to manage their health care.

About MYH:

  • MYH works on prevention, early intervention and youth specialist health response measures regarding adverse childhood events (ACE) and their impact on young people’s health, including speciality programs for young parents
  • This position is located at the clinical health service location of MYH West (which is also the home base for the MYH Young Parents. Program), where multiple support programs run, including the Talking Realities FLO Program and various grant funded Young Parents support Programs.
  • MYH programs at this site, focus on building and strengthening safe and positive relationships for all young people accessing our health services but with special focus on young parents and their children.
  • Building these protective factors improve children and young people’s resilience and reduce the impacts of ACE’s on their future health and disrupting intergenerational cycles.

About You

To be successful in this role, you will have:

  • Appropriate Degree or equivalent qualification which gives eligibility for full membership of the Australian Association of Social Workers
  • Demonstrated high level of ability and professional expertise to relate to and work with young people and their families, who come from diverse social backgrounds related to socio–economic status, race, culture, sexual orientation, abilities, religion, and including those who face a range of complex issues.
  • Provision of specialist assessment, case planning and health care services within a care coordination and case management approach to young people with complex needs. Health care services include engagement, psychosocial support, referral and advocacy, and follow-up.
  • Extensive knowledge of MYH targeted population groups (including Young Parents and Aboriginal young people), relevant counselling and group work aligned to the MYH Model of Care.
  • Extensive knowledge of adolescent psycho-social development, mental health and youth health issues
  • Advanced interpersonal, communication and conflict management skills, including the ability to foster productive working relationships across disciplines, contribute positively to a multi-disciplinary team environment.
