Northern Territory PHN leads the development and coordination of an equitable, comprehensive primary health care system and an engaged health workforce driven by community needs.
ABOUT THE OPPORTUNITY
The Integrated Care Lead is responsible for enhancing the patient journey through local health systems.
In close collaboration with stakeholders, the role is focused on key areas of health system integration including optimising referral pathways for patients, enhancement of multidisciplinary care, strengthening systems of communication between health care providers and supporting clinicians with health system navigation.
Leading a small team, the Integrated Care Lead is responsible for the design, development and ongoing management of programs and initiatives that support best-practice care and health system navigation.
The role is required to understand how the Australian health systems works and the interoperability between different parts of that system.
ABOUT YOU
Formal training; relevant tertiary qualification or extensive experience in a health system management role. Formal training in communications, project management and/or stakeholder engagement. Clinical background and/or strong knowledge of clinical primary care is advantageous.
People Management: demonstrated experience in staff management, senior-level stakeholder engagement, project/program management and budget/financial management.
Team Management: demonstrated experience in team management with ability to mentor other staff to and to foster a culture of high-performance.
Project Management: excellent project management skills throughout project lifecycle, including the ability to engage effectively with stakeholders and deliver complex projects within scope, budget and timeframes.
Communication skills: excellent communication skills, both written and verbal.
Australian Primary Health Care System: demonstrated experience in and/or knowledge of the relevant health care system.
Understanding or demonstrated ability to learn about models of multidisciplinary health care, allied health and value-based care.
Working autonomously, under pressure, exercising initiative and evidence-based judgement.
Demonstrated critical thinking with advanced problem-solving skills and the ability to recognise opportunities for service improvement.
Excellent engagement skills with a proven ability to build collaborative relationships with a wide range of internal and external partners and stakeholders.
Demonstrated ability to thrive in a dynamic and rapidly changing environment.
