Killarney Memorial Aged Care trading as K-Life is seeking a passionate and experienced health professional to enhance clinical governance, strengthen case-managed care, and drive improved health outcomes for the community.
Funded under the federal Innovative Model of Care (IMOC) Grant, this role is responsible for ensuring the grant meets its objectives by integrating case management, multidisciplinary care, and community health initiatives to improve outcomes.
Reporting to the Chief Executive Officer, you will support and mentor nurses, ensure clinical governance aligns with best-practice models, and lead initiatives that enhance health and wellbeing across the community.

Key Responsibilities
Provide clinical governance to ensure clients receive case-managed multidisciplinary care that exceeds best-practice standards.
Oversee the IMOC Grant, ensuring all initiatives align with funding criteria and improve community health.
Work closely with the Community RN and Medical Centre RN, ensuring a consistent holistic approach to clinical care.
Lead initiatives that improve health outcomes, including wellness, education, and preventive health programs.
Support integration of care between the Medical Centre and Community Services to improve access and coordination for those with complex needs.
What We’re Looking For
✔ Registered Nurse, Public Health, or Healthcare Management qualification.
✔ 2+ years’ experience in clinical governance, healthcare leadership, or a related experience.
✔ Understanding of case management and person-centred care models.
✔ Strong knowledge of aged care, disability services, and primary healthcare systems.
✔ Excellent leadership and communication skills to support clinical teams and stakeholders.
✔ Passion for improving health outcomes in rural communities.
