About the Role:
Experienced Healthcare Professional – Disability Support
Are you passionate about making a difference in the lives of people with disabilities? We are seeking a compassionate and skilled healthcare professional to join our dynamic team. If you have a strong background in patient care, chronic disease management, and disability support, this role is for you!
You’ll assist with daily living activities, personal care, medication supervision/administration, and training support workers where relevant, all while respecting the dignity, privacy, and independence of the individuals you support.
You need to be knowledgeable and experienced in the following:
Enteral Feeding and Management
Tracheostomy Management
Urinary Catheter Management
Ventilation Management
Subcutaneous Injection Management
Severe Dysphagia Management
Complex Bowel & Stoma Management
Diabetes Management
High Risk Seizure Management
Complex Wound Management
Key Details:
Applicants must be willing to work across multiple shift times, including mornings, afternoons, evenings and weekends.
Casual role with the potential to transition to permanent employment.
Applicant is to work a minimum of 5 hours a day.
Responsibilities:
Conduct comprehensive assessments to improve patient outcomes.
Domestic duties (cooking, cleaning, meal preparation).
Develop and implement Disease Management Care Plans.
Assisting with daily living activities and ensuring a safe and supportive environment.
Provide holistic physical, psychosocial, and emotional care for patients.
Administer medications and perform diagnostic services, including ECG, spirometry, stress tests, and hearing tests.
Educate and train support workers and carers in providing complex health care.
Work collaboratively within a multi-disciplinary team to achieve better health outcomes.
Complete shift notes and other documentation as required.
Taking individuals for appointments when needed.
Communicate NDIS policies and ensure compliance with safety regulations.
Building a trusting and positive relationship with the client to enhance their quality of life.
Available to work weekdays and weekends.

What You’ll Need:
Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA).
Bachelor of Nursing or equivalent.
Current Australian Driver’s license and a reliable vehicle
Current NDIS Worker Screening Check.
First Aid and CPR Certificate.
National Police Check.
Experience as a registered Nurse.
Experience as a Support Worker (preferred but not essential).
Strong communication skills and a genuine passion for helping others.
