About the role

Join the dynamic team at Your Voice Pty Ltd as a Support Coordinator / Psychosocial Recovery Coach. This full-time role is based in Darra, QLD and is an excellent opportunity to make a meaningful difference in the lives of individuals with disability and complex support needs.

What you'll be doing

Coordinate the delivery of tailored, person-centred support for individuals with disability, mental health conditions, and complex support needs

Collaborate with clients, their families, and support networks to develop and implement comprehensive support plans

Facilitate access to community-based services, resources, and programs that promote independence, social inclusion, and overall wellbeing

Provide psychosocial recovery coaching to empower clients to achieve their personal goals and increase their capacity for self-management

Advocate on behalf of clients to ensure their needs are met and their rights are protected

Maintain detailed client records and contribute to the continuous improvement of service delivery

What we're looking for

Relevant tertiary qualification in social work, psychology, or a related field

Experience not necessary but higher chance to fit into the role faster in a similar support coordination or psychosocial recovery coaching role

Strong understanding of the NDIS, mental health support services, and the needs of individuals with disability

Excellent communication and interpersonal skills, with the ability to build rapport and collaborate effectively with diverse stakeholders

Demonstrated problem-solving, conflict resolution, and negotiation skills

Compassionate and empathetic approach, with a genuine commitment to empowering and supporting your clients

Proficiency in written and verbal communication, with strong organisational and time management skills

Willingness to travel within the local area and work flexible hours as required
