Sunflower Services is seeking a dedicated and compassionate Senior Support Coordinator/Psychosocial Recovery Coach to join our dynamic team.
The Senior Support Coordinator plays a key role in ensuring Sunflower Services provides a high-quality Coordination and Psychosocial Recovery Coaching service to NDIS Participants, supporting them with expertise and warmth to achieve their goals. This includes both managing a reduced caseload of NDIS Participants, and supervision and oversight of a team of Support Coordinators. The Senior Support Coordinator will be a dynamic, forward-thinking individual who thrives in a solutions focussed environment. They will contribute to the continuous improvement of our Support Coordination service, ensuring that the team is equipped with the latest knowledge of NDIS legislation and changes. The Senior Support Coordinator’s exceptional leadership will inspire others, cultivating a culture of collaboration and excellence within the team.
As a Senior Support Coordinator/Psychosocial Recovery Coach at Sunflower Services, your responsibilities include:
Collaborate with the Operations Lead to ensure adherence to KPI policies, manage team performance, and provide support to meet weekly targets. Track billable hours for COS/PRC staff and take necessary follow-up action.
Develop and implement personalised support plans for a reduced caseload of participants, connecting them to relevant services.
Allocate new referrals to team members within 24 hours and ensure timely follow-up. Collect referral and cease data, and utilise for continuous training and improvement.
Lead, mentor, and supervise the Support Coordination/PRC team, providing training, guidance, and regular performance reviews. Encourage constant team growth through continuous skill development and support.
Ensure adherence to company policies, NDIS guidelines, and performance consistency. Monitor team compliance.
Facilitate monthly meetings to collaborate, discuss updates, improvement points, reporting standards etc.
Identify signs of burnout and implement strategies to support staff wellbeing.
Monitor, record, approve and manage team Leave. Provide case load cover when a team member is on Leave.
Address participant and stakeholder complaints, using feedback to improve service delivery.
Facilitate team meetings, encourage a positive and collaborative environment, and support colleagues during leave to maintain continuity.
Contribute to team and company growth by suggesting new ideas, reflecting Sunflower Services' values in actions, and demonstrating leadership by example.
Ensure accurate recording of enquiries, referrals, and ceases in tracking systems. Stay updated on NDIS policy changes and ensure team compliance with these standards.
Monitor KPI compliance and case notes each week to ensure ongoing compliance. Each week record data accurately, track patterns and ensure prompt follow up with relevant team members. Implement meetings, additional education, supervisions, and Performance Improvement Plans where required. Implement due dates and incentives to ensure compliance documentation is consistently completed.
Participate in leadership meetings, contributing to recruitment, policy development, and strategic planning. Ensure the team meets goals and maintains high service standards.
To succeed in the role, you will need:
Tertiary qualification in a relevant field, or significant experience working in the Community Services sector (e.g., Disability, Mental Health, Housing, Justice).
Social Work, Disability Education, or other Allied Health qualifications/experience are advantageous but not mandatory.
Extensive experience and knowledge of the NDIS, with a particular focus on Support Coordination and promoting choice and control for Participants.
Relevant management experience is highly desirable.
Strong interpersonal skills with the ability to establish rapport, build trust, and maintain boundaries.
Excellent communication skills, both verbal and written.
Knowledge of community resources and social services.
Empathy, compassion, and a non-judgmental attitude.
Ability to work collaboratively within a multidisciplinary team.
Proficiency in documentation and record-keeping.
Ethical and culturally sensitive approach to client care.
