About the Role

Croften Care is seeking an experienced Support Coordinator / Psychosocial Recovery Coach to join our passionate team in Rose Park, SA. In this full-time role, you’ll provide high-quality, person-centred support to individuals with complex needs—helping them work toward their goals and lead more independent, empowered lives.

What You’ll Be Doing

Coordinating access to supports such as housing, employment, education, and community services

Leading multidisciplinary teams to deliver targeted, person-centred interventions

Providing one-on-one coaching to help clients build life skills, confidence, and resilience

Advocating for clients while liaising with government and community stakeholders

Keeping accurate, compliant client records and documentation

Attending regular team meetings, supervision, and training opportunities

What We’re Looking For

A strong understanding of the NDIS and its frameworks

Experience supporting people with psychosocial disabilities, mental health concerns, and/or complex needs

Excellent communication and relationship-building skills

A strengths-based, client-focused approach to problem-solving and decision-making

Strong organisational and time management skills

Confidence using digital systems and maintaining high-quality documentation

A current driver’s licence and willingness to travel locally
