What You’ll Be Doing

• Chatting with clients (mostly over the phone) to understand what support they need to live well at home
• Helping them plan and coordinate the services they want on their terms
• Checking in regularly, updating plans, and making sure things stay on track
• Guiding clients through their budgets and options - we’ll train you on all of this
• Building genuine relationships, earning trust, and being their go-to support
• Logging the important stuff and making sure it’s all up to standard
• Facilitating client support by recruiting, engaging, and coordinating with independent subcontracted care providers to meet your clients’ needs

You’ll Thrive in This Role If You

• Can handle a few spinning plates! As in you’re organised and on top of things
• Can hold thoughtful conversations, especially when they’re personal or tricky
• Know your way around a computer and are willing to learn new systems
• Are curious, kind, and not afraid to take initiative

(Experience in aged care or case management is a bonus, but not essential. We’ll teach you what you need to know.)
