About the Role
We are seeking an experienced and visionary Allied Health professional to step into the role of Allied Health Coordinator within the CALHN Integrated Care Geriatric Services team. This key leadership position supports all Allied Health Professional (AHP) and Allied Health Assistant (AHA) roles across our dynamic integrated care programs. You’ll work closely with multidisciplinary teams, hospital and community services, and non-government organisations to enhance care continuity, promote early discharge, and reduce unnecessary hospital admissions for older adults.
In addition to maintaining a clinical caseload, you will provide strategic coordination, mentoring, and clinical supervision across the service. As a recognised expert in your field, you’ll lead initiatives including the Single Point of Contact (SPOC), champion the use of virtual care and digital solutions, and facilitate multidisciplinary case conferences. Your leadership will be pivotal in shaping innovative, patient-centred models of care across CALHN’s integrated services.
About You
You are a passionate and experienced Allied Health professional with a strong commitment to delivering high-quality, patient-centred care in a complex, fast-paced environment. Your ability to lead, mentor, and collaborate across disciplines ensures excellent outcomes for patients and teams alike.
Exceptional communication and clinical reasoning skills, with the ability to manage complex caseloads independently and as part of a multidisciplinary team.
Demonstrated leadership in fostering collaboration across diverse teams, navigating change, and driving quality improvement initiatives in healthcare settings.
Extensive experience working with adults across acute, sub-acute, and community services, including managing patients with neurological, cognitive, orthopaedic, and psychosocial complexities.
Strong organisational and problem-solving skills, with a flexible, culturally competent, and evidence-based approach to patient care and service delivery.
