Join our dynamic, supportive and flexible team to provide clinical and preventative health services to priority communities in the ACT region. The Aboriginal Health and Aged Care Team is multi-disciplinary team of Nurses, Aboriginal Health Workers, Aboriginal Outreach Workers and Care Coordinators, supported by a variety of Specialists and Allied Health Workers. About the position We are looking for a Care Navigator to join our Aborginial Health and Aged Care team. As a Care Navigator, you will support the delivery of health and aged care services to vulnerable communities through improving access to community based aged care, care navigation and care coordination. Work as part of a team to support the community to promote community engagement, care navigation and coordination, health education, health appointments and clinics that contribute to support the wellbeing of the individual and community. Based in Tuggeranong, this position is a full-time opportunity, available to 30 June 2026, with the possibility of extension. Full-time positions have the option to enjoy a Rostered Day Off each month. We are looking for someone with:

  1. Current qualification in a Human Services relevant field such as social work, community services, aged care or disability care, or Cert III (minimum) in Individual Support (Ageing).
  2. Relevant experience in the aged care, health, disability, or related human services sector and an understanding of the Australian aged care system, including My Aged Care processes, different types of services (home care), eligibility criteria, and relevant legislation and policies.
  3. Experience in providing case management support, including assessment, planning, coordination, and monitoring of services, and demonstrated cultural competence.
  4. Ability to work independently and confidence in exercising clinical judgement.
