Affinity Health Solutions is a specialist community nursing organisation seeking Registered Nurses who are reliable, compassionate and committed to working with amazing and diverse people in our community.

We operate business hours Monday - Friday. Times and days are flexible.

You will start in a casual position for 3 months after which there will be a performance review, and an offer for permanency discussed.

The working week is split between working from home or the office, and visiting clients in their own homes. You will be using your advanced clinical skills and knowledge to conduct health assessments, provide care plans and coordinate case management as part of a multidisciplinary team. You will work closely with clients living with disabilities and complex care needs, and their families, to support the building of capacity for self care and achievement of their goals, whilst remaining at home in the community .

The role requires you to be able to work autonomously using critical thinking skills. You need to be able to adapt to clinical changes in the community and apply well developed interpersonal, negotiation and conflict resolution skills to escalate medical concerns appropriately, whilst supporting a clients right to choice and control.

We are an NDIS registered service provider for Perth metro, regional and remote areas of WA. We also provide expert nursing services to privately funded clients, Department of Health, Justice and other organisations.

Suitable applicant/s will need to have:

Advanced skills and knowledge in clinical assessment and care planning

Your own vehicle for all work related travel within the metro area. (all work related travel is a tax deduction).

Effective strategic planning skills to manage complex physical and mental health care needs.

Exceptional English speaking, reading and writing skills

Strong clinical assessment and report writing skills

Strong critical thinking and problem solving skills

Self-motivation and accountability

The ability to work autonomously as well as within a team is essential

Exceptional organisational and multitasking skills

Intermediate to advanced proficiency with various computer software e.g. word/excel

Reliability and a strong work ethic

Essential Criteria

Evidence of current registration by the Nursing and Midwifery Board of Australia.

Demonstrated clinical experience conducting health assessments, providing care plans for identified health issues, and delivering evidence-based nursing care.

Demonstrated well developed interpersonal, negotiation and conflict resolution skills as both an individual and as a member of a multidisciplinary team.

Demonstrated experience working autonomously using critical thinking skills either in a clinical area or community setting.

Demonstrated excellent written and verbal communication skills.

5+ years post-graduation.

Current unrestricted driver’s license and access to own vehicle.

Current knowledge of legislative obligations for equal opportunity, disability services and occupational safety and health, and how these impact on employment and service delivery.

Desirable

Experience in the field of Community Nursing.

Clinical experience in Case Management of clients with a range of health conditions.

Clinical experience in a rehabilitation framework for Spinal Cord Injuries, Autism Spectrum Disorders, Intellectual Disabilities, Mental Health conditions, Neurological conditions including Early Onset Dementia and Aged Care.

Knowledge of the National Disability Insurance Scheme (NDIS) and experience in producing comprehensive nursing reports.

Demonstrated education and training skills, including the ability to provide onsite training and assessment of support workers.
