Your New Role:
This full-time position comes with a salary that reflects your experience (because we know you're worth it!). In this role, you'll be at the heart of everything we do, managing and supporting our amazing team of support workers, ensuring our participants get the high-quality care they deserve. Your responsibilities will include:
Personalised Care Planning: Work with participants to create care plans that truly reflect their individual goals and needs.
Team Leadership: Lead, support, and guide our support workers with kindness and empathy, ensuring they deliver excellent care.
Record-Keeping & Compliance: Stay on top of essential paperwork and industry standards (because we know that stuff matters).
Proactive Problem-Solving: Take the lead on finding solutions when challenges arise, working collaboratively with participants and staff.
Effective Communication: Build strong relationships with participants, families, and stakeholders to keep everyone in the loop.
Service Enhancement: Constantly find ways to improve our processes and service delivery (because there's always room to grow).
What We're Looking For:
Experience in disability care coordination or a related field (bonus points if you’ve already got a bit of a track record).
Strong communication skills (clear, confident, and compassionate).
Knowledge of the disability sector and relevant regulations.
Top-notch organisational skills and a knack for problem-solving.
A genuine passion for improving the lives of people with disabilities.
