The Role

As a Support Coordinator, you'll play a vital role in connecting participants with their NDIS-funded services. You'll collaborate closely with participants, their families, and a range of service providers to help individuals make the most of their NDIS plans. This is a meaningful and diverse role where you'll truly make a difference—empowering people to exercise choice and control while supporting their journey toward greater independence and capacity building.

Key Responsibilities

Support participants in understanding, navigating, and implementing their NDIS plans.

Link participants with appropriate service providers that align with their individual needs and goals.

Coordinate a range of supports across multiple services to ensure seamless and high-quality delivery.

Empower participants to build confidence, independence, and self-management skills in handling their supports.

Offer continuous support by tracking progress and assisting with plan reviews and necessary adjustments

What We’re Looking For

A genuine enthusiasm for supporting individuals with disabilities.

Proven experience in support coordination and implementing NDIS plans.

A relevant qualification in Disability or a related field.

Outstanding communication and interpersonal skills, with the ability to foster trust and strong relationships with participants and their families.

A strong commitment to person-centred support, focused on empowering participants to build independence.

A proactive mindset with strong problem-solving abilities.
