About the role

As a Case Manager at Rebound Rehab, you will play a vital role in supporting injured workers and participants on their journey to recovery. This full-time hybrid role (work from home and within community) with our registered office space being in Liverpool, NSW 2170

What you'll be doing

Coordinating and managing the care and rehabilitation of scheme participants to facilitate their recovery

Conducting comprehensive assessments to identify barriers and develop tailored rehabilitation plans

Liaising with key stakeholders including medical providers, community case managers and scheme agents to ensure effective communication and case management progression

Monitoring progress and adjusting rehabilitation plans as needed to optimise outcomes

Providing regular updates and reports on case progress to all relevant parties

Communicating the needs of participants and ensuring they receive the appropriate support and resources

Maintaining accurate and detailed case records in line with legislative and organisational requirements

What we're looking for

Relevant tertiary qualification in Rehabilitation Counselling, Occupational Therapy, Nursing, Social Work or Psychology disciplines

Extensive experience (5+ years) in case management or a similar role working with persons with significant injury (Traumatic brain injury, Amputation or Spinal Cord Injury)

Demonstrated ability to effectively liaise and negotiate with a diverse range of stakeholders

Strong problem-solving and decision-making skills, with the ability to think creatively

Excellent communication and interpersonal skills, with a compassionate and empathetic approach

Proficient in case management principles, rehabilitation processes and relevant legislation

Highly organised with strong time management and the ability to prioritise a demanding workload
