Position Overview:

This position reports to the Program Manager.

The role of a Support Coordinator/Psychosocial Recovery Coach is to provide individualised support to National Disability Insurance Scheme (NDIS) participants who have a mental illness and or disability. You will work with the participant to implement the supports within the persons' plan for the purpose of building capacity, managing the impact of their disability and improving their health, well-being and participation within the community.

You will provide varying levels of coordination and transition support for young people and adults, considering individual circumstances and level of complexity. Participants will be matched with community-based and/or mainstream programs to fulfil their individual needs whilst receiving continuous support and assistance.

You will be a valued member of the Wise Life Services team and contribute to the development of service practice by providing regular feedback to management and participating in team discussions. All service delivery must be in line with the policies and procedures and the requirements of the NDIS Act and the NDIS Quality and Safeguards Commission.

You are responsible for supporting an NDIS participant to implement all the supports within their NDIS plan including informal, mainstream and community, as well as funded supports.

You will support the participant to strengthen their ability to coordinate their own supports and participation in the community along with ensuring mainstream services meet their obligations, for example housing, education, justice, and health.

Key Responsibilities:

Support Coordinator/Psychosocial Recovery Coach, will be responsible for providing the following service delivery requirements:

Providing support to participants in accordance with their NDIS plan and goals.

Ensure regular contact/monitoring is maintained and recorded.

Maintain appropriate case notes and indicators of progress within required time frames.

You will collaborate with the broader service system and will assist with the coordination of NDIS and other supports.

Service Agreements are in place for all participants

Ensure the participant is in informed about service provider feedback processes.

Work with participants to achieve their goals in line with their NDIS plan.

Work with key stakeholders to support participants in implementing their plans, including LGBTI and Migrant Support Services where applicable.

Participants will be assisted to take more control of their lives and to better manage complex challenges of day to day living.

Through strong and respectful relationships and skilled coaching, participants will be supported to build capacity, including strengths and resilience.

Recovery coaches will work collaboratively with participants, their families, carers and supports to design, plan, implement and adjust a recovery plan.

Providing support to participants in accordance with their NDIS plan and goals.

Provide a quality service to people with mental health needs in their preferred setting, as identified through their recovery assessment.

Commitment to the special needs of children and vulnerable people with a disability; from a culturally or linguistically diverse background; and/or from an Aboriginal or Torres Strait islander background.

Providing relevant information to people with mental health needs about programs they may benefit from.

Role Requirements:

Police Check - A national police check completed within the last 3 months. An updated police check must be supplied to the employer every 3 years.

Registration to Work with Vulnerable People (RWVP) - A valid work with vulnerable people check must be held by all employees that is NDIS endorsed.

Required to undertake Safeguarding Children and Abuse Prevention Programs prior to working directly with children or vulnerable people.

Right to Work in Australia - Australian or New Zealand Citizenship or relevant working visa documentation must be supplied.

Required to undergo an employment health assessment which allows Wise Life Services to be aware of potential and actual health issues for employees and (if appropriate) to work with individuals to ensure we manage these appropriately (e.g. tailoring the work environment to cater for specific requirement/s).

Conform to WLS’s conditions of employment, Code of Conduct, Confidentiality, Anti-Discrimination, Bullying and Harassment policies, Work Health and Safety policies including signing a personnel declaration.

This position is mandated by law to report all current concerns about the safety, welfare and wellbeing of a child.

Report any suspected abuse or neglect of children, adults with a disability or ageing person.

Complete NDIS Commission worker orientation module ‘Quality, Safety and You’.

Carry and use Wise Life Service’s Identification Card.

Competency in using cloud-based IT applications including case management systems and Office 365; including Outlook, Word and Excel.

Access to a computer and a mobile phone.

Selection Criteria:

The incumbent will possess demonstrable competency, knowledge, understanding and/or skills in

the following areas:

Prior experience developing comprehensive assessment and planning techniques.

Ability to meet key competencies associated with supporting people to set and reach their goals.

Ability to use a creative approach to the design of goals and skill development programs with people experiencing mental health needs.

Demonstrated skills in actively engaging with people who are reluctant to use support services.

Excellent organisational, time management skills, and ability to be self-directed.

Ability to work with people with challenging behaviour, supporting them in their home and in the community.

Effective written and verbal communication skills and able to write accurate and comprehensive reports.

Able to work within complex environments and be outcome focused.

Essential Qualifications:

Minimum Certificate IV in disability or community services, or a relevant tertiary qualification in social work, psychology, mental health or the social welfare related field or equivalent.

Required to hold a Tasmanian Working With Vulnerable People Card (WWVP) that is NDIS Endorsed.

Award: Social, Community, Home Care and Disability Services Industry Award MA000100 at Level 5 for Support Coordination, Level 4 for Psychosocial Recovery Coaching.

Desirable:

Two years’ experience in a similar field.

Vehicle License - A valid basic class C full Australian driving license.

Mental Health First Aid

Competency in using the range of Microsoft Office applications.

Level of Responsibility:

Responsible for managing time, planning, and organising their own work, ensuring shift duties, and completed.

Participate in regular supervision.

Willingness to receive and act upon constructive feedback from supervisors and stakeholders.

The Recovery Coach/Support Coordinator is accountable to the Program Manager for all aspects of service delivery whilst at work.
