About the Opportunity

Join our Older People’s Mental Health (OPMH) Community Teams, a vital part of the South Western Sydney Local Health District (SWSLHD), and help provide compassionate, recovery-oriented care to older adults in need across Macarthur and Wingecarribee.
About the Role
As part of the OPMH Community Team, you will support older adults experiencing mental health challenges, whether they are living independently, with family, or in residential aged care facilities (RACF). Your role will focus on delivering person-centred, recovery-oriented biopsychosocial care planning and interventions, ensuring that every individual receives the support they need to thrive.
We’re looking for professionals with experience in assessment, care planning, and interventions tailored to older adults experiencing mental ill health. Previous experience in a care coordination role will be highly beneficial.

What You'll be Doing
The Mental Health Professional is responsible for providing evidence-based, strengths focused, recovery orientated assessment and treatment within a multi-disciplinary team and coordinating consumers’ recovery plans. The role is also responsible for non-clinical tasks including participation in quality improvement activities, clinical supervision and participation in administrative responsibilities as required.

They work closely with inpatient services, GPs, and other community organisations to provide holistic care.

- Relevant qualifications as a Registered Nurse, Occupational Therapist or Psychologist currently registered with the Australian Health Practitioner Regulation Agency (AHPRA), or qualified Social Worker eligible for registration with the Australian Association of Social Workers (AASW), or CSU Graduates: Bachelor of Health Science (Mental Health).

- Using an example, describe a time where you have had to work as part of a team to solve a problem. Outline the communication skills used and how you ensured the problem would not occur again.
- Applicants must possess an Australian C-Class driver's license (including P1 or P2). Those who hold a C class license (P1 or P2 included) from a state other than NSW must obtain a NSW C class (P1 or P2 included) license within three months of appointment. Learner's licenses are not permitted.
