About the role
The Primary Care Liaison Officer (PCLO) is an identified position that will work as part of the Primary Care Practice Liaison Team and is responsible for engaging with and supporting Primary Health Care Providers in a range of PHN activities aimed to support locally relevant, effective, and efficient health care services for the patient population.
The role will be responsible for delivering initiatives aimed to supporting general practice businesses meet current standards, adapt to changing priorities, maintain awareness of best practice principles across the primary health care sector, including for integrated care, digital health, opportunities for partnerships, networking, clinical and non-clinical team development, models of care, and as sought by the Department of Health and Aged Care.
The PCLO assists the strategic objectives of the PHN by supporting Primary Health Care providers to improve equitable access to high quality health care for all people in our communities. This role is a key link to Primary Health Care providers to improve the regional health system.
The successful candidate will have:
- Identify as being Aboriginal and/or Torres Strait Islander and be an active member within your community.
- Advanced interpersonal and relationship management skills including a positive attitude, emotional intelligence, collaboration, decision making, networking, motivation, teamwork, collaboration, negotiation.
- High level of organisational skills
- Understanding of primary health care and current health care trends and community issues in the Australian context
- High-level computer literacy with a range of software packages including but not limited to Microsoft Office Suite.
- Support Practices to strengthen their capacity and capability to deliver culturally responsive care.
- Work with our communities and health providers to improve access to services, health literacy and support the management of health conditions for First Nations people living and working in our region.

Additional Information
**Salary: This role is classified as Grade F – J within our Enterprise Agreement and the rate will be dependent on skills and experience/qualifications. New appointments are generally between grade F-G.
