Ability Choice Care is an innovative and energetic disability organisation working to improve the lives of people with disability throughout the Western Sydney. We offer a range of services including a “State of the Art” Activity Centre, innovative community supports and modern Supported Independent Living houses, in quality environments.

The Role: Senior Coordinator

The Senior Coordinator will demonstrate higher level skills and an excellent knowledge of disabilities, NDIS processes, person centred supports and contractual requirements. The Senior Coordinator performs a critical role, ensuring that participant plans are analysed, broken down to address plan goals and managed and costed to cover the stated plan period.

To succeed in this role, you will need great diligence and demonstrated skill in communicating with a range of participants and stakeholders. A broad range of disability knowledge, across physical, sensory, mental health and intellectual domains is essential, so that supports are maximised through strong evidence. This will be in the form of clinical reports and recorded observation.

An excellent standard of literacy, numeracy, and communication skills will be expected from the successful applicant,

Reporting to the Business Manager you will:

Duties:

Manage and Coordinate supports for an allocated caseload of NDIS participants.

Organise support shifts through liaison with the Logistics (Rosters) Officer and use of the company rostering system (Shift care).

Work closely with the Logistics Officer, providing oversight and support of their caseload.

Hold regular caseload review meetings with the Business Manager and Logistics Officer

Maximise funding and participant retention through effective communication and formulation of evidence. I.e., For scheduled reviews and Changes in Circumstances.

Driving and formulating effective plan delivery, supports and advocacy for participants with complex needs.

Address and resolve complaints from clients about supports and non-compliance matters. For more serious matters, escalate to Senior management immediately.

Identify potential staffing shortages and inform internal management so recruitment can proceed accordingly. I.e., recruiting field and/or Head Office staff.

Establish and maintain relationships with all stakeholders including employees and client families, being aware of client needs, preferences and allocating staff accordingly, to meet those needs.

Attend community, family and stakeholder meetings as directed.

Attend NDIS, T and G, peak organisation and representative meetings as directed by the Business Manager

Document staffing issues and feedback continuously to management, improving our service quality.

Ensure the pursuit and achievement of plan goals. These support services will be delivered in a manner consistent with the values of Ability Choice Care.

Mentor, guide, provide feedback and directives on a day-to-day basis to support workers and service participants.

Consult with work colleagues, provide feedback to the Business Manager and develop relationships with external stakeholders including client’s families, nominees, allied health professionals and fellow support coordinators.

Record and deliver metrics consistent with individual plans and stated goals.

Day to day financial monitoring with particular focus on plan capacity, duration and types of services articulated in the plan breakdown.

Track client health and wellbeing, ensuring early intervention as appropriate, briefing of support staff, follow up and setting of face-to-face appointments with advocates, stakeholders, and participants, as necessary.

Perform other admin duties as required (e.g., attending to phones, answering emails, assisting with interviews.

Selection Criteria
You will show genuine passion for managing key functions in the business through:

· Demonstrating your experience in coordination of supports, effectively addressing the needs of participants with disability whilst adhering to company values.

· Providing behavioural examples of how you have effectively managed a dedicated caseload to achieve outcomes, with individual responsibility and accountability.

· Giving examples of how you have been able to resolve complex participant issues, through liaison with allied health professionals, LACs or NDIA representatives.

· Describing how you would minimise financial losses, shift cancellations, and would generate excellent participant/support worker feedback.

· Collaborative teamwork and affective oversight of direct reports and support staff.

· Achievement of negotiated KPI’s

· Providing examples of where you have used innovation and resourcefulness to improve service to participants.

· Displaying values and attitudes that reflect a high level of integrity, passion, and reliability.

· Being a fearless advocate and enthusiastic pursuer of social justice.

· Supporting people with complex presentations including intellectual disabilities, complex health, and physical disabilities

· Bringing plan goals to fruition through effective costing, planning, negotiation, communication and assessment of participant goals and unmet needs.

Mandatory Requirements:

· possessing relevant qualifications and/or experience in Community Services/Case Management (or equivalent). Social Science or Allied Health qualifications/experience is highly desirable.

· possessing a Current Car licence, Comprehensive Car Insurance, Working with Children Check and First Aid Certificate.

· completion of mandatory NDIS Induction modules and certification.
