About Mawarnkarra Health Service (MHS)
Mawarnkarra (MHS) is the largest primary health care provider providing direct services to Aboriginal and Torres Strait Islander peoples living in the Roebourne area. MHS provides the families of Roebourne with a fully functional and accredited Clinic that treats approximately 3,000 people per year.

The Position
The role of Patient Journey and Transport Coordinator is to promote access to health services by ensuring medical appointments, including specialists' appointments that are made for patients through to the safe transport of patients to and from their appointments in and around the West Pilbara region and to metropolitan centers such as Perth.
The Transport coordination aspect of the role is to lead the Mawarnkarra' s transport team to ensure that clients have transport to health services & medical appointments including specialists' appointments in and around the West Pilbara region & metropolitan areas such as Perth.

The Requirements
Motivation to provide service excellence.
High level of scheduling experience
Ability to work independently and in a team environment.
Excellent oral and interpersonal communications skills.
Ability to manage a crisis situation
Liaise with the Senior Clinic Receptionist, Patient Transport Officers, Registered Nurses and Doctors, when necessary, in regard to patient's medical appointments and Ensure patients understand the appointment time and place the appointment is being held at.
Ability to prioritise workload and demonstrated organisational skills.
Excellent time management skills & have high level of organisational skills.
Coordinate team of Patient Transport Officers and manage team accordingly.
Assist with the transport of patients as required.
Ability to provide a high level of customer service in all dealings with clients.
Live locally to the Roebourne area and be willing to travel to and from daily.
Maintain and upkeep patient progress and notes in internal system, Communicare.
Other requirements
Must have functional computer skills
Experience working in an Aboriginal Medical Service or co-operation (desirable)
Must hold a Passenger Transport Driver Authorization (PTD) or consent to undergo the medical and subsequent application with the Department of Transport. Employment is conditional pending the outcome of this authorization.
Consent to undergo a Working with Children Check and a National Police Certificate. Employment is conditional pending the outcome of these checks.
Must hold a current Australian Drivers Licence
Have reliable transport
Current CPR Certificate
All employees are subject to regular random drug and alcohol screening.
Remuneration and Benefits include:
Attractive hourly rate of $42.88 + superannuation
11.5% Superannuation
No weekend work, Monday to Friday only
