Duration: Full time, fixed term until 30 June 2026

Base Salary: $78,695 to $85,992 per annum (HPCS 3

This position is identified for an Aboriginal and/or Torres Strait Islander person and is intended to constitute a special measure under section 8 (1) of the Racial Discrimination Act 1975 (Cth), and section 57 of the Anti-Discrimination Act 1996 (NT).

Connect with local Elders, older Aboriginal and Torres Strait Islander people and their families to raise awareness of their aged care entitlements.

Who are we?

Established in 1991, Danila Dilba Health Service (DDHS) is an Aboriginal community-controlled organisation committed to providing culturally appropriate, comprehensive primary health care services and community development within the Biluru (Aboriginal) community of the Yilli Rreung (Greater Darwin) Region in the Northern Territory.

We work to address the social determinants of health including the broader issues that affect our people's health, through evidence-based research and public advocacy. We also work to empower individuals, families, and communities to optimise their health.

About the role

Play a vital role in supporting clients mental, emotional, and cultural well-being in the Greater Darwin Region.

Responsibilities:

  • Provide high-quality services to Aboriginal and Torres Strait Islander adults who have been referred and accepted to DDHS following a psychosocial distressing situation/crisis within moderate acuity.
  • Initiate, coordinate, and support the planning, development, and implementation of culturally appropriate interventions, including client postvention activities, and develop individually tailored care and safety plans.
  • Provide 3-month case coordination and an additional 1-month follow-up. check-ins as required to support people to remain linked with their appropriate support networks to facilitate ongoing care.
  • Monitor and assess client's status to implement up-to-date changes to care plans to ensure appropriate supports are in place.

To be successful you will need:

Essential

  • Identify as Aboriginal
  • Cert IV Mental Health/Cert IV in Aboriginal and/or Torres Strait Islander Primary Health Care or equivalent training/ work experience or lived experience.
  • Ability to work independently and in a team environment.
  • 2 or more years of relevant work experience
  • Knowledge of the social determinants of health (environment, economic and social inequalities) that have a profound impact on the health and well-being of Aboriginal and Torres Strait Islander people.
  • Demonstrated ability to use culturally sensitive ways to ensure improved client access to DDHS and/or other health services.
  • Proven teamwork, communication, and interpersonal skills with the ability to constructively negotiate outcomes with a diverse range of people and service providers.
  • High level of personal resilience and flexibility with the ability to deal with challenging situations.
  • Demonstrated computer skills, especially in the use of client information systems or similar.

Desirable

  • Experience working in community, hospital, or primary health care setting with clients with a range of presentations and needs.
  • Experience working in Aboriginal community-controlled sector.

What we offer:

  • 6 weeks annual leave per annum
  • 5 days paid leave during end of year shutdown (on top of annual leave).
  • Salary packaging of up to $15,990 per annum.
