The Institute for Urban Indigenous Health Ltd was established by its founding members to provide a coordinated/integrated approach to the planning, development and delivery of comprehensive primary health care services for Aboriginal and Torres Strait Islander populations within the Southeast Qld Region.
The focus of IUIH Aged Care team is to work with the local Indigenous community and relevant key stakeholders to increase access to comprehensive care and provide well-coordinated, quality in home care services to eligible aged care clients.
About the Role?
The position is responsible for leadership and management of Aged Care Operations for the Wide Bay Region including business partner / stakeholder engagement, client engagement and communications, business, contractual and financial management. The position also oversees day-to-day Aged Care Operations ensuring the integrity and consistency of application of the model and the quality and efficiency of service delivery. The position will also lead the implementation of continuous improvement and change, including significant Aged Care Reform program.
Within this role, you will be required to:
- Provide performance development and functional support to all IUIH and contracted sites in the delivery of best practice implementation in line with the IUIH model of Care
- Liaise and consult with target Aboriginal and Torres Strait Islander communities and health services to collaboratively assess health needs and priorities and subsequently facilitate appropriate Chronic Disease medical specialist service delivery arrangements that address identified issues.
- Lead the planning and implementation of Aged Care and disability activities in line with contracted program requirements
- Lead the ongoing development, review, evaluation and continuous improvement of in-home aged care and disability service delivery systems and processes
- Provide functional support in addressing service delivery issues and build capacity in line with the approved Model for service delivery
- Coordinate an effective quality system and framework ensuring compliance with all relevant legislation including ISO, Food Safety Act and Home Care Standards
- Develop a team in the establishment and implementation of operational policies and procedures, client contracting and assessment processes and implementation of required software and systems processes and tools
About You?
- Current C Class Drivers Licence (QLD)
- Current Criminal History Check
- Current Disability Worker Screening Check (Yellow Card) – required prior to your commencement
- Current Working with Children Clearence (QLD Blue Card) – required prior to your commencement
Desirable, but not mandatory:
- Demonstrated experience working with Aboriginal and Torres Strait Islander people and /or communities
