The Disability Health Support (DHS) Coordinator is required to assess level 3 supports and develop health care plans for HCSA participants and to provide relevant training (theory and practical skills) to support workers and promote best practice and high quality.

Disability Health Support Coordinator Roles and Responsibilities
The roles and responsibilities of a Disability Health Support Coordinator may include, but are not limited to, the following:
• The ability to provide and have the knowledge in providing disability-related health supports under NDIS, including dysphagia, nutrition, diabetes, continence, skin integrity support and training, medication management, epilepsy, Peg -feeds and other health related needs as they arise to meet the needs of participants.
• Identify and respond to the learning needs of individuals and the services required to perform tasks and activities required to safeguard and promote health and wellbeing.
• Develop participant’s healthcare plans in conjunction with participants and stakeholders of Level 3 services to ensure adequate services are provided. Ensure all plans are clear and appropriate in planning with the implementation of clear training to support staff of individual health care requirements.
• Complete assessments at different locations (workplace and participant's home).
• Train new employees as part of the induction process, and modify all level three training materials as best practice and changes occur, including all mandatory medical training plans such as
medication administration
infection control
continence care
• To ensure employees have adequate instructions to perform their duties safely and effectively and to provide quality practical assistance and training for level 3 supports to ensure support worker compliance.
• Ongoing supervision of supports centred around level 3 requirements to ensure compliance and practices are attended to in alignment with NDIS Guidelines and best practice.
• Liaise and promote stakeholder engagement with medical professionals such as General Practitioners, Medical specialists, hospitals and clinics to promote best practice outcomes for participants and support needs.
• To assist with the investigation and management of any medication-related incidents, to ensure incidents are managed in an effective and timely manner. Appropriate actions must be provided to staff accordingly.
• To implement and utilise nursing skills in the delivery and support to a person with a disability and their support staff.
• To manage Holistic Care SA online assessment platform to ensure workers are compliant with their training.
• To provide 1:1 training and supervision for support staff who require extra support and guidance in appropriate practice standards.
• Record case information, complete accurately all necessary forms and documentation
• Perform other duties as assigned.
• Source and evaluate externally delivered learning and development services as required.
• Develop, coordinate and ensure the delivery of meaningful training initiatives engagingly and interactively with a clear, measurable outcome
• Keep accurate and complete records of the ‘supportability’ of your work activities in accordance with legislation and operational requirements and liaise with and report to the NDIA and associated organisations when required.
• Ensure Work Health and Safety guidelines are adhered to, and all industry-relevant legislation and codes of conduct are complied with
• Maintain an up-to-date knowledge of the NDIS systems and practices.
Disability Health Support Coordinator Accountabilities
• Provide ongoing planning and education services to support staff in line with the NDIS guidelines and standards.
• Undertake work in line with the organisation's policies, procedures and practices
• Maintain accurate and up-to-date case notes on participant progress
Essential Qualifications & Experience
• Disability Health Support Coordinator with APHRA registration as a Registered Nurse
• Extensive experience in the design, development and continuous improvement of learning initiatives and programs
• Demonstrate awareness of the organisation’s vision, mission, and values
• Demonstrate high-level interpersonal and communication skills.
• Well-developed oral and written communication skills
• Ability and confidence to deliver training to various groups
• Commitment to the delivery of a quality participant service in balance with WHS principles, cultural diversity, and equal employment opportunity
• Demonstrated understanding of the changing and diverse needs of home care participants
• Ability to work with a high level of independence and autonomy
• Ability to manage, record and adapt to participants needs in regard to Hospital discharges, and making sure it is a smooth transition from Hospital back to home
Desired Experiences
• Tertiary qualifications and registered with APHRA for a minimum of 2 years of experience
• Previous learning and development experience within the health or not-for-profit sector
Minimum Knowledge
The Disability Health Support Coordinator must understand the relevant legislative requirements, in particular
NDIS Code of Conduct
Understanding of NDIS
Work Health and Safety Act 2012 (SA) (the WHS Act)
Work Health and Safety Regulations 2012 (SA) (the WHS Regulations) and supported Codes of Practice

Personal Abilities/Aptitudes/Skills
• Demonstrated high-level interpersonal/communication and analytical skills

• Willingness to work in a busy and challenging environment
• An understanding of cultural sensitivity
• Proven experience in the use of Microsoft Office 365 Application, including Outlook, Word, Excel and Sharepoint
• Proven capability to allocate time effectively and efficiently
• Demonstrated understanding of the changing and diverse needs of home care participants
• Strong customer service skills and an ability to apply problem-solving skills to a range of situations
• Ability to process and prioritise a high volume of activities in a fast-paced environment
• Demonstrated ability to use a planned, structured approach to allocation of resources
• Ability to work with a high level of independence and autonomy
