About the Department
Palliative Care is the active total care of people whose disease is not responsive to curative treatment. Care is delivered by coordinated medical, nursing and allied health services that are provided, where possible, in the environment of the person's choice. Currently, inpatient palliative care is provided at Caritas Christi, Kew with 16 inpatient beds. The Palliative Care Consult Service (PCCS) supports patients in the acute care facility at Fitzroy, works within St Vincent's Private Hospital and at St George's Hospital Kew.
About the Role
The Clinical Nurse Consultant/Team Leader is responsible for the smooth and efficient functioning of the Palliative Care Consult Service (PCCS) available to clinicians, patients and families within St Vincent's Hospital Melbourne, St Vincent's Private Hospital, St George's Hospital and other affiliate organisations.
The Clinical Nurse Consultant/Team Leader will work closely with Palliative Care Medical Consultants and Lead Physician and is required to represent palliative care at relevant internal and external stakeholder meetings. They are responsible for coordination of administration, rostering, supervision and clinical activities including operational leadership of nursing within the Palliative Care Consult Service
The Clinical Nurse Consultant/Team Leader will coordinate the provision of palliative care consults and advice, provide a clinical lead to health professionals across the hospital network, and manage the resource utilisation and support of clinical projects. This includes the timely referral of patients to inpatient palliative care units, including Caritas Christi.
It is essential to have highly developed interpersonal, time management and communication skills with the ability to work effectively both independently and with the multidisciplinary team.
Classifications for this position will range from ZA7 - ZA8 ($119,324 - $122,163 per annum)
Duties
- Assessing and reviewing patients to ensure they are appropriate for palliative care services
- Assessing patient care needs and family care needs
- Liaising with community palliative care services
- Assisting in facilitating transfers to inpatient palliative care
- Educating and mentoring clinicians throughout SVHM, SVPH, St George's Hospital and its affiliates.
- Promoting the role of the PCCS and maintaining a high profile, both internally and externally as appropriate
- Responsible for coordination of administration, rostering, supervision and clinical activities including operation leadership of nursing within the Palliative Care Consult Service
- Facilitate, lead and participate in meetings including Palliative Care Consult Service, Family Meetings and Multidisciplinary Team Meetings
Qualifications, Skills and Experience
- Degree in general nursing and current registration with the Australian Health Practitioner Agency;
- Post graduate qualification in palliative care
- Minimum of five years full time equivalent, post-registration experience in palliative care nursing
- All appointments are made subject to the candidate providing a current satisfactory National Police Certificate conducted by The Victoria Police.
