We are currently looking for an experienced Support Coordinator to join a small team focused on quality outcomes. You will assist and support NDIS participants, their families and carers to enable them to implement their NDIS Plan and achieve their goals and aspirations.

Role:

• identifying and connecting with appropriate providers of supports to achieve the objectives of the participants NDIS plans
• working along with other allied health staff, as well as other service provider stakeholders to support the professional and efficient service delivery in a way that is responsive to the needs of the clients
• providing participants with an independent person to discuss their concerns and goals, assisting to access support when required and pre-plan for future
• continuing professional development in accordance with the NDIS requirements

Day to day you will:

• Support participants, their families and their formal/informal support network to understand their NDIS plan
• Complete and engage in regular communication / reports with the participants and their families and NDIA
• Identify the most appropriate services within the community to suit an individual's needs, support participants to communicate their goals and aspirations to their stakeholder’s team and the NDIA
• Attend home and community visits with participants when permitted
• Undertake general case management of participant portfolio’s
• Manage and monitor individuals NDIS plan budgets

Requirements:

• previous Support Coordination experience is a must
• Thorough knowledge of NDIS
• Do you have Case management experience and great time management skills?
• Hold a valid Working with Children and Disability card?
• Current driver license

A Support Coordinator assists these people to make the best out of the funding they receive in a care coordination/ management way. You may be surprised to know we support people with a broad range of issues including significant work in the mental health, corrections and child safety space, enabling them to implement their NDIS Plan and achieve their goals and aspirations.
