About the role

As the Coordinator of Supports (CoS) at Ontrack Tasmania, you will play a pivotal role in supporting individuals with disability or age-related needs within the Glenorchy, Tasmania community. This full-time position will see you working closely with clients and their families to coordinate access to essential services and resources, ensuring they receive the assistance they require to live independently and with dignity.

What you'll be doing

Assessing client needs and developing tailored support plans to address their specific requirements

Liaising with a wide range of service providers, including healthcare professionals, government agencies and community organisations, to coordinate the delivery of essential services

Advocating on behalf of clients to ensure they receive the appropriate level of care and support

Regularly reviewing and updating client support plans to adapt to changing needs

Maintaining detailed records and reporting on client progress and outcomes

Providing guidance and emotional support to clients and their families

Collaborating with the wider Ontrack Tasmania team to continuously improve service delivery

What we're looking for

Experiance within the Community sector and NDIS is recommended

Extensive experience in a similar client-facing role, preferably within the aged care or disability support sector

Strong communication and interpersonal skills, with the ability to build trust and rapport with clients from diverse backgrounds

Excellent problem-solving and decision-making abilities, with a focus on delivering positive outcomes for clients

Proficient in the use of relevant software and technology to effectively manage client records and coordinate service delivery

Demonstrated commitment to upholding the rights and dignity of individuals with disability or age-related needs
