The Role
Centrecare is seeking a compassionate Mental Health Nurse or Registered Nurse with experience and an interest in mental health to join our Integrated Primary Mental Health Care (IPMHC) team as a Clinical Care Co-ordinator. This position is available as a part-time or full-time (0.6 – 1 FTE) role. The successful applicant will play a significant role in providing accessible, responsive, high-quality, and integrated community mental health services for individuals experiencing severe, persistent, and complex mental health conditions.
Working closely with the Program Manager and multidisciplinary team, you will coordinate clinical care, contribute to comprehensive assessments, and provide evidence-based interventions that promote client wellbeing and recovery.
A Few Specifics

  • Delivering clinical assessments and supporting medication management in collaboration with GP
  • Providing ongoing care coordination and treatment planning for clients
  • Offering education and support to clients and their families
  • Facilitating referrals and shared care arrangements with external services
  • Maintaining accurate client records using the Penelope database
  • Supporting and guiding team members as part of a multidisciplinary mental health team
  • Promoting an integrated approach in line with Centrecare’s Therapeutic Framework and the WA Primary Health Alliance (WAPHA) stepped care model.

About You

  • Registration with AHPRA
  • Tertiary qualifications in nursing or a relevant discipline
  • Experience in mental health, clinical assessments, and case management
  • Ability to build trusting relationships with clients, families, and general practitioners
  • Excellent written and verbal communication skills
  • Strong computer literacy and record-keeping abilities
  • Experience working across cultures, particularly with Aboriginal and Torres Strait Islander peoples
  • Ability to work autonomously and collaboratively in a team environment
  • A current Working with Children Check or ability to obtain one
  • A current National Police Clearance
  • A current WA "C" class driver’s licence and access to a vehicle
