About the Team
The HSP aims to build the skills and knowledge for the social and economic well-being of clients. It delivers Settlement Services to Clients through a tailored, needs-based case management approach. The relationship with the Case Manager is the foundation on which the HSP case management process is based, working in partnership with Clients and other service provider personnel, case workers, and professionals involved in supporting the Client. HSP Clients will be classified into a Tier according to their level of need and the complexity of their circumstances. The majority of HSP clients are expected to be classified under Tier 1 and Tier 2 and will receive the standard HSP services. Client displaying an inability to independently engage with appropriate supports and who might be impacted by multiple and complex barriers may be classified under Tier 3 following written approval from DHA. Tier 3 clients will receive specialised and intensive HSP services.
About You
You will provide holistic case management services under the Humanitarian Settlement Program (HSP) to newly arrived refugees and humanitarian entrants (Clients), including those with complex needs requiring specialised and intensive services, to build the skills and knowledge for social and economic well-being. The Case Manager will be the central point of contact and assume overall and ultimate responsibility for the management of the delivery of Settlement Services to Clients. The Case Manager will provide HSP services to Tier 1, Tier 2, and Tier 3 Clients. We have multiple locations across Sydney.
Responsibilities:
- Develop a holistic and client-centred case plan with the involvement of the clients and consistency with identifying needs. Implement case management plan and continuously monitor, review, and update the plan as required.
- Case management activities, including service coordination, advocacy, case conferences and liaison with other support services and agencies, and ongoing support of clients and families with highly complex needs.
- Support clients through individual crises via crisis management principles, information provision, and referrals to specific support services
- Delivering orientation sessions on various settlement outcome domains to the newly arrived humanitarian entrants under the Community Support Program

Essential Criteria
- Bachelor / Masters of Social Work or Psychological Science or Psychology
- Experience providing case management support to clients with complex needs
- Experience working with databases and Microsoft Office
- Worked with clients from a culturally and linguistically diverse background
