The Role

Nurse Next Door is on the hunt for a superstar Care Designer (Registered Nurse) who will design, evaluate and oversee our clients care plans in working towards the common goal of making their lives better to ensure they are safe and happy in their own homes. In addition, this role comes with the opportunity to lead a team of caregivers and nurses, who are passionate about making a difference.

Operating in a home care environment, you will be ensuring that our clients are safe and happy in their own homes. You will be responsible for managing an existing portfolio of clients and staff, as well as meeting with potential clients to set up individualized care plans to meet their unique needs.

Reporting to our Chief Clinical Officer, your role as part of our leadership team will be to build our client base through business development activities, clinically support and oversee the care of our current clients and oversee the management of a team of caregivers and nurses. You will do this with the support of our multiple Care Ambassadors (who act as Clinical team leaders and are the frontline managers for staff and clients), and a supportive Administration and Operations Team. Duties will include but will not be limited to the following:

Meet with potential clients to consult on their options for premium home care services.

Design tailored and meaningful client care plans that foster positive relationships between our clients, the Care Team and their external health professionals and stakeholders

Support our clients with delivering Nursing and Caregiving visits

Oversee client clinical care of low and high intensity clients and maintain meticulous care documentation records

Be accountable for maintaining the high clinical standards set by Nurse Next Door Brisbane, following the documented policies and procedures including incident management

Oversee the management and development of a growing team of caregivers and nurses.

Build professional relationships with relevant stakeholders in the health industry

Why We’ll love you

7 years of experience as a Registered Nurse

Previous community care experience in the field of NDIS and Aged Care

Proven previous experience leading a clinical team

You will have current experience/knowledge in wound care, catheterisation, medication management, diabetes management and manual handling practices

You are a self-led leader with the ability to take responsibility for your behavior and success.

You have a proven track record in meeting KPI's and promoting success of the team.

You are exceptionally well organized, demonstrate impeccable time management skills, and possess excellent verbal and written communication skills

You enjoy networking and building relationships with stakeholders across the sector

You will be able to work in a fast paced environment where you are flexible, adaptable and able to respond easily to last minute changes or challenges

You will be genuinely compassionate, and show passion about making a difference in the lives of those you meet

Be proficient in the use of technology across different software systems and different applications.

Job Requirements

AHPRA Registration as a Registered Nurse

Up-to-date skills in catheterisation, wound care management, diabetes management and manual handling practices (continence care desirable)

Strong interpersonal skills and a service-first mentality

Strong time management and organizational skills

Technologically savvy with mobile apps, desktop applications and G-suite

Requires some flexibility and availability to work occasional evenings and weekends on-call as required

CPR & First Aid Certification

Current QLD Drivers License (with no restrictions)

National Police Check (Aged Care/ Vulnerable Persons Sector)

NDIS Worker Screening Check

NDIS Worker Orientation Module Completion

Working with Children Check
