About Your Role

This position is responsible for the efficient and effective operation of the local Transition Aged Care Program. The program is embedded in sites across Murrumbidgee Local Health District (MLHD), and serves to transition aged care clients safely from hospital, and either back into the community or into residential care, depending on client / carer need and preference.

The Transitional Aged Care Coordinator ensures that local program therapies, services, documentation, communication, and administrative requirements, meet the needs of the client, and relevant MLHD, NSW Ministry of Health, and Commonwealth Transition Care guidelines.
