We are seeking a Support Specialists (Disability Support Workers) who are compassionate about delivering high-quality person-centered care to the people they support. There is a preference for experienced workers, with previous exposure providing high-quality person-centered care to the people they support. Positive Futures Queensland is a clinically led practice that offers a range of different services to people who are situated across South East Queensland, and we are seeking supports workers which can service one of or more of the following areas: South East Brisbane and North Brisbane. We are excited to welcome friendly and passionate people onto our team.

ABOUT THE ROLE

We are seeking empathetic and resilient Support Workers for permanent, part time or casual opportunities to work morning, afternoon, overnight and on weekends. You will be working with a range of different individuals including people experiencing complex mental health, challenging behaviours and/or cognitive decline.

Key Responsibilities

Personal care and domestic assistance (light cleaning)

Community access and transportation

Maintain and monitor person centred plans

Documenting and progress note keeping

Develop rapport and strong relationships with the people we support and their families

KEY RESPONSIBILITIES

Work collaboratively within a team.

Assist clients in Community Access environments to develop independent living skills

Assist clients to develop and / or expand their social relationships & networks

Follow and participate in person centered support planning

Work with a compassion and creativity to increase a person’s quality of life

Complete detailed shift notes and incident reports.

Provide direct support to people with a disability with all activities of daily living which may include domestic duties, personal care, social support, medication assistance, manual handling, transport, meals and attending appointments.

Establishing and maintaining positive relationships with all key stakeholders; clients, families, guardians, and involved services.

KEY REQUIREMENTS FOR ALL APPLICANTS

A current NDIS Worker Screening Check or valid exemption

Current First Aid & CPR training

Fully Vaccinated

Have a vehicle to enable travel to and from participants and stakeholders (travel is reimbursed).

PREFERRED EXPERIENCE/QUALIFICATIONS

Administration of medication training (or willing to obtain)

Manual handling training (or willing to obtain)

Those who have strong work ethics and enjoy being a team player

Understanding of confidentiality within the NDIS

Reporting and recording relevant documentation for NDIS standard

Knowledge of and awareness of the National Disability Insurance Scheme (NDIS)

Flexibility to work within a service that provides 24/7 care, this includes weekend work, sleepover and /or active night shifts within a fortnightly roster.

Solid experience working with people living with neurodiverse and psychosocial disabilities.
