We are currently recruiting for an experienced Care Coordinator to join a Not-for-Profit organisation on a temporary full-time basis. This role is responsible for providing administrative and logistical support to ensure the efficient coordination of care services for clients.
Key Responsibilities:
- Completing intake and onboarding for new clients
- Communicating with clients and families regarding daily care needs
- Coordinating care services within approved classification and budget
- Managing documentation and compliance requirements
- Conducting monthly check-in calls and service reviews
- Assisting clients with feedback and complaints, ensuring concerns are addressed
- Advocating for client rights and escalating issues as required
Skills & Experience:
- Previous experience in Aged Care or healthcare administration (clinical background not required)
- Strong organisational and administrative skills with high attention to detail
- Excellent written and verbal communication skills
- Ability to manage multiple tasks in a fast-paced environment
This is a full-time temporary role with potential for extension, offering an opportunity to work within a values-driven organisation supporting the community.
