We are looking for casual Registered Nurses for the aged care sector in the inner suburbs of Melbourne.

We are also looking for an experienced Registered Nurse that may be looking to take the next step into a lead role.

You will be responsible for coordinating and planning the care of a caseload of Home Care Clients. You will lead, mentor and coach our extraordinary team of nurses (Registered and Enrolled) and caregivers out in the field. This role is a combination of “hands on” field work, travelling to the client's home, and also requires attendance at our office in Hawthorn.

Reporting to our General Manager, your role will be a combination of front line nursing work, management and development of our growing team of nurses and personal care assistants. This includes but will not be limited to the following:

Working one on one with clients and their loved ones, focusing on their individual nursing needs and happiness

Wound and catheter care

Build, manage and maintain meaningful client care plans that foster positive relationships between our clients and the Care Team

Maintain meticulous care documentation records

Continuous quality improvement

Mandatory Requirements

Registered Nurse (AHPRA Registration required) with no conditions or limitations imposed on practice - minimum 5 years experience

Experience mentoring and coaching a team of Enrolled Nurses and Personal Care Assistants / Support Workers in either a Home Care setting or Aged Care facility is highly regarded

Current Victorian Drivers License

NDIS Workers Screening Check, or willing to obtain

Working With Children Check

The Care Coordinator role is a fulltime position that requires some flexibility and availability to provide on-call clinical phone support as required. Occasional weekend work may be required.
