Care Manager - Aged Care (Relocation Opportunity)
We require an experienced Clinical Manager to manage the clinical care within this medium aged care residence based Swan Hill, VIC.

Representing a values-driven leading NFP aged care organisation, you will work alongside and report to an experienced Facility Manager. This is a well-staffed, large facility that provides a great opportunity for learning and development.

The Role:
As a CCM, you will drive the leadership of clinical care to all residents and become an important leadership team member.

• Medium sized home
• Home is running well with strong occupancy levels
• Overseeing a lovely team of Registered Nurses and PCAs
• Happy to consider a candidate with strong aged care knowledge stepping up into the role

Requirements:

• Demonstrated Care Manager experience in the aged care sector
• Valid AHPRA Registration (DIV 1)
• Extensive clinical skills and knowledge.
• Highly developed leadership, communication, and change management skills.
• Working knowledge of the requirements of the Quality Agency Standards
• Demonstrated organisational skills with accuracy and attention to detail.
