Job overview

- Conduct thorough initial assessments to develop individualised care plans tailored to each client's specific health requirements

- Monitor client progress regularly, adjusting care plans as needed to ensure effective interventions and positive outcomes

- Administer medications, treatments, and therapies, following strict safety protocols and accurate documentation procedures

- Perform specialized nursing procedures such as wound care and catheter care, adhering to best practices and clinical guidelines

- Educate clients and their families on disease management, medication administration, and preventive health measures to promote self-care and well-being

- Collaborate with an interdisciplinary healthcare team and maintain effective communication with clients, families, caregivers, and other healthcare professionals to coordinate care, optimize service delivery, and ensure seamless client transitions and continuity of care

- Demonstrate organization values, a deep understanding of Aged Care Quality Standards, and adherence to the Aged Care Code of Conduct.

- Engage in ongoing professional development to stay current with healthcare advancements, enhancing the quality of care provided

Multilingual Female candidates will be a preference for this role with the ability to speak fluent English and be conversational in both Arabic and Assyrian.

Requirements for the job

· Current registration with AHPRA as a Registered Nurse

· Bachelor's degree in Nursing (mandatory)

· Proven experience as a Registered Nurse in home care, community health, or aged care

· Knowledge of relevant aged care regulations and standards

· Current First Aid and CPR certification

· Valid Australian driver’s license

· Up-to-date COVID-19 vaccinations (minimum 2 doses, unless medically exempt)

· Current National Police Clearance (within 12 months)

· NDIS Worker Screening Check (if applicable)

· Fluent in English; conversational in Arabic and Assyrian

· Membership with a professional nursing body (e.g., ACN) – desirable

· Manual Handling and Infection Control training – desirable
