Service Manager – Disability Sector (Full-Time)

About the Role: We are seeking an experienced and passionate Service Manager to oversee and manage services for ten (10) participants, including Supported Independent Living (SIL) and Community Access Support (CAS) participants. This is a full-time role that requires a dedicated leader with strong case management skills, a deep understanding of the NDIS framework, and a commitment to delivering person-centered care. Participants are spread across Southeast Queensland, requiring site visits and stakeholder meetings at least once a fortnight.
Key Responsibilities:
Participant Management: Oversee the coordination of support services for 10 participants, ensuring their individual needs, goals, and NDIS plans are met.
Staff Leadership: Lead, mentor, and support a team of Disability Support Workers, promoting best practices and person-centered care.
Service Coordination: Develop, review, and implement individualized support plans in accordance with NDIS guidelines.
Compliance & Quality Assurance: Ensure all services meet NDIS standards, organizational policies, and industry best practices.
Stakeholder Engagement: Build strong relationships with participants, families, guardians, and external providers to facilitate seamless support.
Rostering & Scheduling: Coordinate and manage staff schedules to ensure effective service delivery while maintaining operational efficiency.
Incident & Risk Management: Identify, report, and manage risks, safeguarding the well-being of participants and ensuring service continuity.
Budget & Funding Oversight: Monitor participant budgets and NDIS funding allocations to optimize service provision.
Documentation & Reporting: Maintain up-to-date records, including case notes, incident reports, and service progress reports.
Continuous Improvement & Growth: Identify loopholes in the current network and collaborate with the board to brainstorm strategies for service expansion and execution.
Site Visits & Stakeholder Meetings: Conduct site visits and attend stakeholder meetings at least once a fortnight to ensure continuous improvement in service delivery.
What We’re Looking For:
Experience: Minimum 2 years in a leadership role within the disability, aged care, or community services sector.
Qualifications: Diploma or higher in Disability, Community Services, Social Work, or a related field.
Knowledge: Strong understanding of the NDIS framework, SIL processes, and person-centered support models.
Skills: Excellent leadership, communication, problem-solving, and organizational skills.
Compliance: Current NDIS Worker Screening Check, Blue Card, First Aid & CPR, and a valid Driver’s License.
