We are seeking a dedicated and compassionate Service Manager to join our team. In integral part of the Executive Team, the Service Manager plays a crucial role in providing effective and efficient operations and delivery of MHA Cares Home Care Services. This will be achieved by effectively leading, motivating and developing teams in the delivery of Consumer Directed Care while enhancing our customer’s quality of like and well-being and achieve the strategic direction of the organisation within designated clinical and business frameworks.

Duties and responsibilities:

Oversee the assessment, coordination, and delivery of HCPs, CHSP, HACC PYP and FFS, to ensure services are tailored to individual client needs.

Manage the implementation and coordination of MHA’s HCP, CHSP, HACC PYP and FFS

Oversee the Development and implementation of person-centered care plans based on client assessments, goals, and preferences.

Oversee the care management and brokerage services to older people to meet their needs in a manner that enhances their rights, dignity, and independence.

Oversee the effective and flexible provision of services to clients and their families within agreed budgetary limits.

Oversight and management of the Intake, CHSP, HACC PYP, HCP and Service Delivery teams

Lead, mentor, and support a team of case managers, support workers, and administrative staff to ensure high-quality service delivery.

Ensure all aged care services comply with relevant government regulations, funding requirements, and accreditation standards.

Prepare reports on service performance, compliance, and funding requirements for senior management, board and regulatory bodies.

Oversee the allocation of resources within HCPs, CHSP, HACC PYP and FFS to ensure services are delivered efficiently and within budget constraints.

Manage funding agreements and reporting requirements in line with government contracts.

Provide a timely, accurate advisory service to the CEO and Board on statutory, community, technical and organisational aspects of the Care Services functions.

Undertake a selection process to recruit staff on merit who demonstrate the skills, attitude, personal attributes and values aligned with MHA’s culture and values.

Action the established policies and procedures of MHA as expressed through Board meetings, the MHA Strategic Plan and Budget.

Network and develop relationships with key community and industry stakeholders as they relate to the organisation (e.g. referrers, client representative groups, other service providers, industry groups and professional bodies).

The successful applicant will have:

• Tertiary Qualification in a related health field i.e., Registered Nursing.

• Current registration with AHPRA or other professional clinical body with evidence of recent practice.

• Proven management and leadership experience in a care environment for 5+ years.

• Strong communication, problem-solving, and interpersonal skills.

• Proficiency in client management systems and Microsoft Office Suite.

• Valid driver’s license and ability to travel as required.

• Proven ability to use sound judgement and experience to address challenges efficiently or escalate them appropriately.

• Dedicated to delivering person-centered care and supporting the independence and dignity of aged and vulnerable people.

• Driven to achieve meaningful outcomes for both clients and the wider community.

• Capable of working both independently and collaboratively within a multidisciplinary team environment.

• Highly skilled in managing time effectively and staying organised in a dynamic setting.

• Team-oriented with a strong ability to establish and nurture effective professional relationships.

Desirable:

• Nursing experience in aged care, community care or a related function

• Knowledge of Home and Community Care Services

• Knowledge of new Aged Care Act and current reforms
